Agreement for participating in muscle activation and movement coaching. You accept the following agreement upon booking an appointment with Alto Performance.

 

The 'Activity/Activities' refers to the participation in muscle activation and movement coaching sessions and general advice given by Alto Performance.

 

I am aware that there are significant risks involved in all aspects of the activity I am participating in. I have voluntarily agreed to participate in these activities and to assume all risk of personal injury, death and property damage resulting from such activities in public or private venues.

 

I agree that:

 

• I have fully disclosed to Alto Performance any injury, condition or impairment which may have a deleterious effect on or be impacted by participating in these activities.

• I am physically capable of participating in the activities and using any equipment associated with such activities.

• I have parents or guardians consent if I am under 18 years old.

• I have been given permission to participate in the activities by a physician or have chosen to participate without approval of a physician.

 

I am willingly and voluntarily participating in the activities of Alto Performance. I hereby release Alto Performance from any and all liability, claims, demands and actions which are related to, arise out of, or are in any way connected with my participation in this activity.
